DefineMD Financial and Consultation Policy
DefineMD is a private, cash-pay medical practice. We do not bill insurance, Medicare, Medicaid, or any state or federally funded medical or prescription benefit programs. All services are paid for directly by the patient.
To receive care through DefineMD, you must be 18 years of age or older and reside in the United States or a U.S. territory.
Patients are responsible for confirming whether their personal insurance may reimburse any portion of their care. DefineMD does not verify insurance benefits, submit insurance claims, or communicate with insurance companies on a patient’s behalf.
DefineMD does not provide services to individuals who are enrolled in government-funded medical or prescription benefit programs. This includes Medicare, Medicaid, Medigap plans, VA benefits, TRICARE, Department of Defense benefits, or any other state or federal medical or pharmaceutical assistance programs. If you have both commercial insurance and any government-funded coverage, you are considered to have government insurance and are not eligible to receive services through DefineMD.
By booking a visit, enrolling in a membership, or receiving care, you confirm that you are not currently enrolled in any government-funded medical or prescription benefit program. If at any point you become enrolled in a government-funded program, you understand that you are no longer eligible to continue care with DefineMD and agree to notify us.
The initial medical visit is a professional clinical service. Payment is required at the time of booking and is non-refundable. This visit includes review of your health history, clinical evaluation, medical decision-making, and care planning. Eligibility for prescription treatment is determined during the visit and is not guaranteed. If you are not medically eligible for a prescription, the visit fee still applies and will not be refunded.
Membership fees cover the medical services included in your selected plan. Membership fees do not include the cost of medications, laboratory testing, imaging, supplements, medical devices, or services provided by third parties unless explicitly stated in writing.
If medications are prescribed, you are responsible for all pharmacy-related costs, including medication pricing, dispensing, and shipping. Medications are fulfilled by independent pharmacies, and pricing and availability are determined by the pharmacy, not DefineMD.
You agree not to seek reimbursement from any insurance plan for services, prescriptions, or out-of-pocket costs associated with care received through DefineMD, and not to apply these costs toward any insurance deductible or out-of-pocket maximum.
If laboratory testing is ordered, you are responsible for all related costs. Laboratory services are provided by independent third-party laboratories, and payment is made directly to the laboratory. DefineMD does not control laboratory pricing and is not responsible for billing disputes related to lab services.
Medical recommendations and care plans are based on clinical judgment, patient-reported information, and applicable standards of care. DefineMD does not guarantee specific outcomes, results, or weight loss.
Memberships provide access to care but do not guarantee availability of specific medications, treatments, or services. Access may be limited based on medical appropriateness, licensing requirements, regulatory changes, or medication availability.
DefineMD may update pricing, services, or policies from time to time. Continued use of services constitutes acceptance of any updated terms.
DefineMD reserves the right to suspend or discontinue services if eligibility requirements are no longer met, payment cannot be collected, required information is not provided, or care cannot be safely or appropriately delivered.
By using DefineMD services, you acknowledge that you have read, understand, and agree to this policy and accept responsibility for all charges related to your care.
